Pay & Hours
Salary: £13.74 per hour
Hours: 35 hours per week
Monday–Friday, 8:45am – 4:15pm

📍Locations
Bassetlaw Hospital (North Notts): Monday & Tuesday
Kingsmill Hospital (Mid Notts): Wednesday to Friday

  • Initially fixed-term, with potential extension depending on funding

🧑⚕️ Role Purpose

This role supports people moving from hospital back into the community, helping ensure safe, smooth transitions and ongoing support.


Key Responsibilities

  • Support patients during discharge from hospital
  • Assess ongoing needs and create support plans
  • Work closely with hospital teams and reablement services
  • Provide presence in Transfer of Care Hubs (TOCHs)
  • Coordinate with volunteers and support staff
  • Ensure person-centred, high-quality support

⚠️ Requirements

  • Enhanced DBS check required (working with vulnerable adults)
  • Must be able to travel across Nottinghamshire
    • Driving licence & own vehicle essential
    • Mileage reimbursed at 45p per mile
