SUPPORT COORDINATORS – SUPPORTED HOSPITAL DISCHARGE SERVICE

  • Salary: £12.50 per hour
  • Hours: Several posts (Part time or Full time available)
  • Area: South Notts        
  • Contract type: Permanent subject to funding

This role encompasses providing essential assistance to patients post-discharge, conducting assessments of ongoing needs, and facilitating the initial phases of their journey beyond the hospital.

Please Note:

An enhanced Disclosure & Barring Service (DBS) police record check in relation to vulnerable adults (previously known as CRB check) is required for these roles.

Ability to travel across Nottinghamshire (and occasionally to other areas if necessary), UK driving licence and use of own vehicle is required for this roles – work related travel expenses are reimbursed at 45p per mile.

HOME SUPPPORT WORKERS

  • Salary: £11.60 per hour plus an additional £1.40 per hour holiday pay on each hour worked
  • Mileage paid at 45p per mile
  • Hours: Flexible
  • Area: Bassetlaw        
  • Closing date: Friday 28th June 2024
  • Interview date: TBC

We are looking for people to join our dedicated team of Home Support Workers who provide domestic support throughout Bassetlaw.
