Join our friendly team at Bassetlaw Action Centre as a Support Coordinator in our Supported Hospital Discharge Service (SHDS).
We have 2 vacancies available as follows:
🕒 Full-time | 37.5 hours/week (5 days) Monday – Friday
💷 £13.34/hour | Permanent (subject to funding)
📍 Based in Queens Medical Centre, Nottingham
🚗 Driving licence and own vehicle required (mileage reimbursed)
🕒 Part-time | 22.5 hours/week (3 days) (Wednesday – Friday)
💷 £13.34/hour | Permanent (subject to funding)
📍 Based in Bassetlaw Hospital, Worksop
🚗 Driving licence and own vehicle required (mileage reimbursed)
💬 What You’ll Do:
✅ Visit patients at home after discharge
✅ Ensure they have essentials like food, medication, and heating
✅ Coordinate support with hospital teams, families, and volunteers
✅ Help people feel safe, supported, and confident at home
✅ Make a real impact in your community every day
🙌 What We’re Looking For:
✔️ Experience in health, social care, or community support
✔️ Great communication and organisational skills
✔️ A caring, flexible, and proactive approach
✔️ Willingness to undergo an Enhanced DBS check (we’ll arrange and pay for it)
